Location: Lebanon, NH

Contract Type: Per-Diem

Shift Type: Per diem (days vary), 3:00 PM - 11:00 PM, Evening shift

Hours per Week: 40

Candidate Pay Rate: $22 per hour

Roles & Responsibilities

Provide compassionate, hands-on care to patients and assist with activities of daily living
Monitor and record vital signs and report any changes to nursing staff
Support residents with mobility, hygiene, and feeding as needed
Document care accurately and timely in the patient record
Communicate effectively with nurses, families, and the care team
Maintain a safe, clean environment and follow infection control practices
Assist with transfers and positioning to promote comfort and safety
Be a dependable member of the team who helps improve patient experience

Qualifications

Active LNA certification (licensed nursing assistant) — VT or NH scope appropriate for the role
Current CPR/BLS certification preferred or willing to obtain
Comfortable assisting with ADLs and basic patient care
Dependable, compassionate, and able to work independently on per diem shifts
Previous experience as an LNA preferred but strong new graduates with certification are welcome

Education

High school diploma or equivalent
Active LNA certification

Knowledge & Skills

Basic patient care and personal hygiene assistance
Taking and documenting vital signs accurately
Safe patient handling and transfer techniques
Clear, respectful communication with patients, families, and staff
Familiarity with charting and following care plans
Understanding of infection prevention and safety protocols
Teamwork mindset and ability to follow clinical direction
