Roles & Responsibilities

• Provide high-quality, compassionate care to patients in the Transitional Care Unit (TCU)
• Perform tracheostomy care and manage tube feeding regimens safely and accurately
• Manage and monitor PICC/PORT lines and NG/G-tubes with attention to infection prevention
• Care for patients with chest tubes and wound VAC systems and escalate changes in condition
• Document thoroughly and timely in Meditech EMR and utilize Pyxis for medication administration
• Collaborate with multidisciplinary teams to create and execute individualized care plans
• Float to Med-Surg and Surgical Pediatrics as needed while maintaining professional standards
• Follow department-specific orientation and training to ensure unit competency and patient safety

Qualifications

• Active State of Hawaii RN license required by assignment start date
• BLS certification required prior to start
• ACLS not required for unit but may be agency requirement — maintain if applicable
• 2–10 years of nursing experience (TCU, med-surg or similar acute care experience preferred)
• Proof of flu vaccination or completed facility-approved medical or religious exemption prior to start
• Comfortable with variable shifts, floating to other units, and flexible scheduling

Knowledge & Skills

• Strong knowledge of Transitional Care Unit nursing and complex patient care
• Proficiency with Meditech EMR and Pyxis medication systems
• Skills in tracheostomy care, tube feeding (NG/G-tube), chest tube management, and wound VAC use
• Competence managing PICC and PORT lines and recognizing line-related complications
• Excellent clinical assessment, critical thinking, and communication skills
• Ability to work effectively in a team and adapt to changing patient needs and shift patterns
