Roles & Responsibilities
- Provide compassionate, high-quality nursing care to residents in a skilled nursing/rehab setting
- Perform thorough assessments and communicate changes in condition promptly
- Administer medications and treatments accurately and safely according to facility policy
- Document care and observations clearly and timely in the resident chart
- Collaborate with nursing staff, therapists, and the interdisciplinary team to create and follow care plans
- Support resident mobility, ADLs, and comfort while promoting dignity and independence
- Participate in shift handoffs to ensure continuity of care
Qualifications
- Current, active Massachusetts LPN or RN license (required)
- CPR/BLS certification (required or must obtain upon hire)
- Previous experience in long-term care, rehab, or skilled nursing preferred
- Strong communication skills and ability to work well on a multidisciplinary team
- Ability to handle night-shift responsibilities and prioritize resident safety
Education
- LPN: Completed LPN diploma or certificate program
- RN: ADN or BSN preferred
Knowledge & Skills
- Medication administration and monitoring
- Basic clinical assessments and documentation practices
- Wound care fundamentals and pressure injury prevention
- Familiarity with electronic medical records/charting
- Time management and delegation during busy night shifts
- Customer-service mindset when interacting with residents and families
