Shift Type: Per diem scheduling (days as scheduled), 8:00 AM - 8:00 PM (12-hour day shift)

Candidate Pay Rate: $42 per hour

Roles & Responsibilities
- Provide compassionate, safe, and evidence-based nursing care to residents in a skilled nursing and rehab setting
- Administer medications and treatments accurately and document care in the medical record
- Perform thorough patient assessments and report changes in condition promptly to the care team
- Collaborate with interdisciplinary team members to develop and update individualized care plans
- Support rehabilitation goals by assisting with mobility, ADLs, and therapeutic activities
- Educate residents and families about care plans, medications, and follow-up needs in a clear, respectful way
- Maintain accurate, timely documentation and ensure regulatory compliance
- Promote a culture of safety, infection control, and resident dignity at all times
Qualifications
- Active LPN or RN license in good standing (appropriate licensure required)
- Previous experience in long-term care, rehab, or skilled nursing preferred
- Current Basic Life Support (BLS/CPR) certification or willingness to obtain
- Demonstrated ability to communicate effectively with residents, families, and care teams
- Up-to-date immunizations and ability to meet facility onboarding requirements
- Dependable, compassionate, and detail-oriented with strong assessment skills
Education
- LPN: Graduate of an accredited practical nursing program (diploma or equivalent)
- RN: Graduate of an accredited nursing program (ADN or BSN)
Knowledge & Skills
- Strong clinical assessment and prioritization skills
- Medication administration and documentation best practices
- Experience with care planning and interdisciplinary collaboration
- Familiarity with infection control and resident safety protocols
- Effective verbal and written communication with residents, families, and staff
- Ability to support rehabilitation goals and assist with ADLs
