Contract Type: Travel Contract
Shift Type: Monday-Friday (business hours), Day shift — options: 7:00 AM - 3:30 PM; 7:30 AM - 4:00 PM; 8:00 AM - 4:30 PM; 8:30 AM - 5:00 PM. Weekends: every other weekend (or 4 weekend days in a 4-week schedule). Day shift, 5x8-hour schedule.
Hours per Week: 40
Candidate Pay Rate: $66 per hour

Roles & Responsibilities
- Coordinate and manage patient discharges to ensure safe transitions of care and timely placement.
- Perform utilization review and case management activities using Milliman Guidelines to support appropriate care.
- Document thoroughly and accurately in Epic to maintain clear, compliant patient records.
- Collaborate with interdisciplinary teams (therapy, pharmacy, social work, interpretation services) to build and execute patient-centered plans of care.
- Conduct needs assessments and arrange DME, community resources, and follow-up care to reduce readmissions.
- Provide compassionate care coordination across the lifespan — from neonates to geriatrics — ensuring each patient’s plan is clear and actionable.
- Float within the case management service area to cover service gaps and support staffing needs.
Qualifications
- AK RN license in hand required for auto-offer (will screen for pending licenses but license in-hand required for auto-offer).
- Minimum 1 year acute care experience (behavioral health experience does not apply).
- Epic (EMR) experience required — Epic hard-stop requirement noted.
- BLS certification required; ACM certification preferred.
- Comfort working in an acute hospital environment with interdisciplinary teams and CMS/HIPAA regulatory expectations.
- Willingness to work every-other-weekend or equivalent (4 weekend days in 4 weeks) and float within case management as needed.
Knowledge & Skills
- Strong case management and discharge planning skills
- Utilization review and familiarity with Milliman Guidelines
- Proficient Epic charting experience
- Care coordination across multiple disciplines and patient age groups
- Good working knowledge of CMS and HIPAA regulations
- Comfort with Microsoft Office and Microsoft Teams for communication and documentation
