Shift Type: Monday-Friday, 7:30 AM - 4:00 PM (Day); 5x8 schedule. Weekends required every other weekend or 4 weekend days in a 4-week schedule. Day shift, with occasional floating within case management services.

Candidate Pay Rate: $66 per hour

Roles & Responsibilities

• Coordinate patient care across teams using Epic to ensure safe, timely discharges and smooth transitions
• Conduct utilization reviews using Milliman Guidelines to support appropriate and efficient care
• Develop and update individualized plans of care, perform needs assessments, and order DME as needed
• Lead discharge planning and collaborate with families and community partners to secure post-discharge resources
• Document thoroughly in Epic while maintaining CMS and HIPAA compliance
• Communicate effectively with interdisciplinary partners to remove barriers and streamline care
• Float within case management services as needed to cover gaps and support the department

Qualifications

• AK RN license in hand required for offer
• Minimum 1 year acute care nursing experience
• Epic EMR experience required
• BLS certification required
• ACM (Case Management) certification preferred
• Comfort working in an acute hospital setting (Magnet, Level II Trauma) and meeting weekend coverage needs
• Candidates must have a permanent address at least 50 miles from the facility (locals not accepted)

Knowledge & Skills

• Proficient Epic charting and EMR navigation
• Utilization review and application of Milliman Guidelines
• Discharge planning, care coordination, and patient advocacy
• Needs assessment and DME ordering processes
• Familiarity with CMS regulations and HIPAA standards
• Comfort with Microsoft Office and Microsoft Teams for communication and documentation
• Strong clinical judgment, organization, and interdisciplinary communication skills
