Roles & Responsibilities
- Provide patient-centered occupational therapy assessments and develop individualized treatment plans
- Implement therapeutic interventions to improve patients’ independence with activities of daily living
- Collaborate with nursing and interdisciplinary teams to coordinate care and support safe discharges
- Document progress and outcomes clearly and timely in the patient record
- Educate patients and families on adaptive techniques, adaptive equipment, and home safety
- Participate in care planning meetings and contribute clinical expertise to improve patient outcomes
Qualifications
- Valid Occupational Therapy license (state licensure required)
- OTR or equivalent credential preferred
- Current CPR/BLS certification
- Comfortable working in a post-acute care setting and delivering compassionate, outcome-focused therapy
- Strong clinical reasoning, documentation, and communication skills
Education
- Bachelor's or Master's degree in Occupational Therapy (OT) or related field
Knowledge & Skills
- Skilled in patient assessment, ADL training, and therapeutic interventions
- Experience with adaptive equipment selection and home-safety recommendations
- Proficient documentation and electronic medical record use
- Ability to collaborate across disciplines and teach patients and caregivers
- Strong time management and caseload prioritization abilities
