Roles & Responsibilities

• Assess residents' mobility, functional abilities, and treatment needs with thorough evaluations
• Develop individualized treatment plans that promote independence and meaningful recovery
• Implement therapeutic interventions such as gait training, therapeutic exercise, and balance programs
• Collaborate with nursing, OT, SLP, and case management to coordinate holistic care
• Document progress clearly and promptly in the facility EMR to support continuity of care
• Educate residents and families on home exercise programs and safe mobility strategies
• Monitor outcomes and adjust treatment plans to maximize functional gains and patient satisfaction
• Participate in team meetings and contribute clinical insight to care plans and discharge planning

Qualifications

• Active Physical Therapist license in California (required)
• Doctor of Physical Therapy (DPT) preferred or equivalent degree and clinical training
• Current BLS/CPR certification
• Experience in skilled nursing, outpatient, or acute care settings preferred
• Strong documentation skills and familiarity with EMR systems
• Excellent communication and teamwork skills; compassionate patient-centered approach

Education

• Doctor of Physical Therapy (DPT) preferred; Master or Bachelor in related field acceptable with appropriate licensure

Knowledge & Skills

• Skilled in therapeutic exercise, manual therapy basics, and gait training
• Knowledge of mobility devices, transfers, and fall prevention strategies
• Ability to perform thorough functional assessments and set measurable goals
• Competence with common modalities and adaptive equipment recommendations
• Effective patient and family education skills
• Comfort working in interdisciplinary teams and communicating with caregivers
