Roles & Responsibilities
- Provide compassionate, resident-centered nursing care at Assisi House
- Administer medications and treatments safely and accurately according to policy
- Monitor and document vital signs, clinical changes, and care interventions
- Collaborate with the interdisciplinary team to support resident goals and safety
- Complete Trinity Specialty Specific Competency Testing and document skills per facility requirements
- Maintain accurate and timely electronic and paper documentation
- Promote dignity, independence and quality of life for residents through thoughtful care
Qualifications
- Active LPN license (practical nursing diploma/certificate) — Pennsylvania preferred
- Current BLS and ACLS certifications
- Completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
- Willingness to complete required health screenings and immunizations (2-step PPD or IGRA, MMR, Hep B, Varicella, Tdap, COVID-19 vaccine, influenza vaccine)
- Ability to pass Trinity 10 Panel Drug Screen and provide a physical/health statement
- Strong communication skills, compassion, and ability to work independently in a per diem pool
Education
- Licensed Practical Nurse (LPN) diploma or practical nursing certificate

- Medication administration and accurate documentation
- Vital sign assessment and clinical observation skills
- Infection control and standard precautions
- Resident-centered/geriatric care principles
- Comfort with Trinity workflows and competency testing processes
- Effective communication with residents, families and care teams
- Adaptability to variable schedules and changing unit needs
