Roles & Responsibilities

• Provide compassionate, resident-centered nursing care at Assisi House
• Administer medications and treatments safely and accurately according to policy
• Monitor and document vital signs, clinical changes, and care interventions
• Collaborate with the interdisciplinary team to support resident goals and safety
• Complete Trinity Specialty Specific Competency Testing and document skills per facility requirements
• Maintain accurate and timely electronic and paper documentation
• Promote dignity, independence and quality of life for residents through thoughtful care

Qualifications

• Active LPN license (practical nursing diploma/certificate) — Pennsylvania preferred
• Current BLS and ACLS certifications
• Completion of Trinity Specialty Specific Competency Testing and Trinity Skills Checklist
• Willingness to complete required health screenings and immunizations (2-step PPD or IGRA, MMR, Hep B, Varicella, Tdap, COVID-19 vaccine, influenza vaccine)
• Ability to pass Trinity 10 Panel Drug Screen and provide a physical/health statement
• Strong communication skills, compassion, and ability to work independently in a per diem pool

Education

• Licensed Practical Nurse (LPN) diploma or practical nursing certificate

Knowledge & Skills

• Medication administration and accurate documentation
• Vital sign assessment and clinical observation skills
• Infection control and standard precautions
• Resident-centered/geriatric care principles
• Comfort with Trinity workflows and competency testing processes
• Effective communication with residents, families and care teams
• Adaptability to variable schedules and changing unit needs
