Roles & Responsibilities

• Ensure patients receive safe, effective medication therapy through clinical pharmacy review
• Prepare, verify and monitor unit dose and IV admixture dispensing with attention to accuracy
• Collaborate with physicians, nurses and the care team to optimize drug therapy outcomes
• Teach and mentor staff and learners as part of the department’s educational mission
• Participate in administrative duties and quality improvement initiatives to strengthen pharmacy services
• Conduct or contribute to research projects that advance patient care and pharmacy practice
• Maintain compliance with all state and federal laws, regulations and hospital policies

Qualifications

• Active pharmacist license in Vermont or eligibility for VT licensure
• Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy preferred
• Previous hospital pharmacy experience required (not retail-only)
• Familiarity with unit dose systems and IV admixture preparation
• Strong computer skills and comfort with electronic medication systems
• Excellent communication skills and the ability to work independently
• Willingness to participate in teaching, administrative projects and research

Education

• Doctor of Pharmacy (PharmD) preferred; Bachelor of Science in Pharmacy acceptable
• Active state pharmacist license required

Knowledge & Skills

• Clinical pharmacy practice with a patient-centered focus
• Drug distribution processes including unit dose workflows
• Preparation and oversight of IV admixtures
• Proficiency with pharmacy information systems and general computer skills
• Technical pharmacy practice standards and hospital pharmacy operations
• Clear, professional communication and self-directed problem solving
