Roles & Responsibilities

Coordinate patient care plans to ensure holistic support and smooth transitions
Conduct comprehensive assessments and develop individualized discharge plans
Provide crisis intervention and therapeutic support to patients and families
Collaborate with interdisciplinary teams to optimize patient outcomes
Advocate for patients' needs and connect them with community resources
Maintain accurate documentation and ensure compliance with regulations

Qualifications

Active LCSW license in California is required
BLS certification from AHA or equivalent is required
5150 certification in California is mandatory (training and exam required)
Prior experience with Dignity Health (CA Dignity) is required
Minimum 2 supervisory references (charge accepted), one within 12 months and one within 3 years

Education

Master's Degree in Social Work

Knowledge & Skills

Expertise in care coordination and discharge planning
Strong knowledge of 5150 holds and involuntary psychiatric evaluations
Excellent communication and interpersonal skills
Ability to work in a fast-paced healthcare environment
Proficiency in electronic health records
