Contract Type: Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM; occasional Saturday rotation once a month

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Assess and evaluate patients' physical therapy needs in a home health setting
Develop and update individualized plans of care in collaboration with physicians
Instruct patients and families in therapeutic exercises and use of equipment
Monitor and report patient progress to the attending physician
Supervise home health aides involved in rehab care
Participate in case conferences and peer consultation
Maintain accurate clinical documentation and comply with regulations

Qualifications

Minimum of 2 years of experience as a Physical Therapist in acute, home care, or rehab setting
Valid BLS certification required
Strong oral and written communication skills
Excellent organizational abilities
Ability to work in Yolo County territories including Woodland, Davis, and surrounding areas

Knowledge & Skills

Knowledge of physical therapy assessment and treatment techniques
Understanding of Medicare, Medi-Cal, OSHA regulations
Proficiency in documentation and clinical record keeping
Compassionate patient-centered care approach
Ability to work independently in a home health environment
