Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Assess patients with compassion and precision to ensure their comfort
Manage pain effectively to improve quality of life
Educate families with empathy and clear guidance during challenging times
Document hospice certifications accurately using Home Care Home Base EMR
Deliver meaningful end-of-life care as a dedicated case manager

Qualifications

Active RN license
Current CPR certification
Valid active Driver's License for mobile visits
2+ years of nursing experience

Education

RN licensure required

Knowledge & Skills

Hospice experience as a case manager
Familiarity with EMR systems like Home Care Home Base
Understanding of hospice certification documentation
Home health experience a plus
Self-sufficient and comfortable working independently
