Roles & Responsibilities

Assess residents using the nursing process and create individualized care plans that support improved outcomes
Administer medications and treatments safely and accurately following physician orders
Document resident status, care provided, and response to interventions in a timely, professional manner
Collaborate with interdisciplinary team members to coordinate care and support continuity
Provide resident and family education to promote understanding and adherence to the plan of care
Participate in performance improvement activities to help elevate quality of care across the facility

Qualifications

Current California Registered Nurse (RN) license
Current certification in CPR
At least 1 year of experience as a Registered Nurse
Graduate of an accredited school of professional nursing

Education

Graduate of an accredited school of professional nursing (required)

Knowledge & Skills

Proficient with the nursing process: assessment, diagnosing, planning, implementation, evaluation
Medication administration and treatment competency
Skilled in patient/resident assessment and documentation
Experience in Skilled Nursing Facility (SNF) care and working with interdisciplinary teams
Ability to interpret diagnostic tests and provide resident education with cultural competency
Familiarity with performance improvement practices and delivering quality-focused care
