Roles & Responsibilities

• Provide compassionate, culturally sensitive diabetes education to patients and families, empowering self-management
• Collaborate with an interdisciplinary diabetes management team to develop and execute individualized patient care plans
• Administer prescribed medications and treatments safely and according to policy and state practice act
• Perform thorough clinical assessments and document care according to standards
• Serve as a resource for patient education materials and help develop patient teaching programs
• Participate in department performance improvement activities and shared governance initiatives
• Utilize critical thinking to promote patient safety and escalate concerns through the appropriate chain of command
• Support continuity of care by offering guidance to other units/departments on the patient’s longitudinal plan

Qualifications

• Licensed RN in Illinois (IL) required; licensure in Wisconsin (WI) noted as relevant
• Graduate of an accredited nursing program required; BSN preferred
• Certified Diabetes Care and Education Specialist (CDCES) required
• Approved CPR certification required within 90 days of hire
• Demonstrated clinical documentation skills and experience with care planning
• Ability to float to other units and collaborate across disciplines
• Strong critical thinking, patient advocacy, and communication skills

Education

• Graduate of an accredited nursing program
• BSN preferred

• Diabetes education and chronic disease self-management strategies
• Clinical assessment and medication administration
• Patient and family education techniques with emphasis on culturally sensitive care
• Interdisciplinary collaboration and care coordination
• Clinical documentation best practices and standards of care
• Performance improvement and shared governance familiarity
