Roles & Responsibilities

• Deliver compassionate, hands-on physical therapy treatments that help residents maximize independence
• Implement and follow the therapist-developed plan of care, including short- and long-term functional goals
• Monitor resident responses and progress, and promptly communicate changes to the supervising therapist
• Document all therapeutic interactions timely and accurately according to department procedures
• Recommend, obtain, and instruct residents and caregivers on appropriate assistive devices
• Provide clear, practical education to residents and caregivers and document teaching outcomes
• Collaborate in interdisciplinary team meetings to coordinate resident-centered care
• Maintain departmental cleanliness, follow infection control procedures, and help with equipment upkeep

Qualifications

• PT Assistant License required
• Associate degree required
• Basic Life Support (BLS) certification required
• Prior experience in long-term care or skilled nursing facility preferred
• Strong verbal and written communication skills; able to document clearly and present information
• Ability to work collaboratively in an interdisciplinary team and attend team meetings
• Capable of performing physical demands of the role, including lifting up to 50 pounds (two-person lift above 50 lbs)
• Committed to resident-centered, compassionate care and meeting Medicare/Medicaid compliance

• Associate degree (required)
