Roles & Responsibilities

Evaluate residents and perform thorough physical therapy assessments
Develop individualized treatment plans focused on functional outcomes
Deliver skilled therapeutic interventions and progress patients toward goals
Document interventions and progress clearly and timely in the chart
Collaborate with nursing and rehab team members to coordinate care
Educate residents and families on home exercise programs and safety
Participate in discharge planning and transitions of care

Qualifications

PT or DPT credential noted as core skills for this role
Open to any experience level, including new graduates
Ability to float occasionally as requested
Professional communication skills and a patient-centered approach

Knowledge & Skills

Strong knowledge of physical therapy assessment and treatment principles
Experience or comfort working in a skilled nursing facility / geriatric setting
Ability to create functional, goal-oriented treatment plans
Good documentation practices and teamwork with interdisciplinary staff
Clinical reasoning and safe patient-handling skills
