Roles & Responsibilities

• Evaluate and treat residents in skilled nursing and post-acute settings to restore function and independence
• Develop and implement individualized treatment plans that target measurable functional gains
• Collaborate closely with an interdisciplinary team to coordinate seamless, patient-centered care
• Document clinical care accurately and timely in accordance with facility and regulatory standards
• Participate in care conferences and family education to align goals and expectations
• Contribute to quality improvement initiatives to enhance outcomes and resident experience
• Provide compassionate coaching and hands-on therapy that empowers residents and builds confidence

Qualifications

• Active California Physical Therapist license (required)
• Strong clinical assessment and intervention skills
• Excellent communication skills for team collaboration and family interactions
• Skilled nursing or post-acute care experience preferred
• Dependable, flexible, and team-oriented approach to patient care
• Commitment to delivering high-quality, compassionate rehabilitation services
