Roles & Responsibilities

Provide compassionate physical therapy services directly in patients' homes across Yolo County
Assess and evaluate each patient's therapeutic needs and create personalized rehabilitation plans
Guide patients and families through exercises, mobility training, and use of therapeutic equipment
Collaborate with physicians and care teams to track progress and adjust treatment goals
Supervise home health aides and coordinate with other care providers for comprehensive support
Document clinical progress and communicate changes in patient condition to the care team

Qualifications

Minimum 2 years of physical therapy experience in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Physical Therapy assessment and treatment planning
Home safety evaluation and patient mobility training
Patient and family education on therapeutic equipment
Documentation and clinical reporting
Medicare and Medi-Cal compliance knowledge
