Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to patients in their homes according to physician plans of care
Assess and evaluate patients' therapeutic needs, muscle strength, mobility, gait, and rehabilitation potential
Develop and update individualized treatment plans while monitoring progress toward goals
Educate patients and families on therapeutic exercises, equipment use, and home safety modifications
Collaborate with physicians and care teams to report changes in condition and adjust treatment approaches
Supervise home health aides and coordinate with other healthcare professionals for comprehensive patient care
Document clinical progress and prepare detailed reports for physicians and patient health records

Qualifications

Minimum 2 years experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong oral and written communication skills with excellent organizational abilities
Valid Physical Therapist license for California practice

Knowledge & Skills

Physical Therapy assessment and treatment planning
Home health rehabilitation techniques
Patient and family education strategies
Therapeutic appliance instruction and prosthetic device training
Medicare and Medi-Cal compliance knowledge
Title 22 and OSHA regulatory understanding
