Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide comprehensive physical therapy evaluations and treatments to home health patients
Develop and implement individualized care plans to restore function and mobility
Educate patients and their families on therapeutic exercises and home safety
Collaborate with interdisciplinary team members to ensure coordinated care
Document patient progress accurately in the HomeCare/HomeBase EMR system
Manage a caseload of 5-7 patients per day within a 60-mile radius

Qualifications

Current North Dakota Physical Therapy License required
BLS certification from American Heart Association
Valid driver's license and auto insurance
Minimum 1 year of home health experience
Proficiency with HomeCare/HomeBase EMR preferred

Knowledge & Skills

Strong clinical skills in orthopedic, neurological, and geriatric physical therapy
Ability to work independently in a home health setting
Excellent communication and patient education skills
Time management and documentation efficiency
