Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide compassionate physical therapy services to home health and hospice patients
Help patients recover from surgeries, falls, and other conditions in the comfort of their homes
Develop personalized treatment plans for 5-7 patients daily
Travel within a 60-mile radius around Valley City to deliver care
Document patient progress using HomeCare/HomeBase EMR system
Collaborate with interdisciplinary teams to support patient goals

Qualifications

Current North Dakota Physical Therapy License required
BLS certification required
1+ year of previous home health experience
Valid driver's license and auto insurance
Strong PT-related clinical skills

Knowledge & Skills

Home health and hospice patient care experience
Comfortable working independently in patient homes
Familiarity with HomeCare/HomeBase EMR
Excellent time management and organizational skills
Compassionate communication with patients and families
