Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every 3rd weekend rotation

Hours per Week: 40

Candidate Pay Rate: $64 per hour

Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes across assigned geographic regions
Develop and implement individualized treatment plans for orthopedic, stroke, and spinal injury patients
Collaborate with PT, OT, and Speech teams to provide comprehensive home-based care
Complete OASIS documentation accurately and on time
Build trusting relationships with patients while helping them regain independence

Qualifications

Active Physical Therapist license in California
Minimum 1 year of clinical experience
OASIS documentation experience required
BLS certification required
Home health experience preferred but not mandatory

Knowledge & Skills

OASIS documentation proficiency
Home-based care approach
Orthopedic and neurological rehabilitation
Cerner EMR system
Patient-centered communication
