Roles & Responsibilities

Conduct physical therapy assessments and interventions for patients in their homes
Develop and implement individualized treatment plans
Collaborate with nursing staff and other healthcare professionals
Perform SOC visits, especially for orthopedic patients
Utilize EMR system (HCHB - HomeCare HomeBase) for efficient documentation

Qualifications

Current Physical Therapy licensure
BLS certification required
Home Health experience preferred, but not required
Ability to perform 5 visits per day

Education

Physical Therapy degree from an accredited program

Knowledge & Skills

Physical Therapy techniques and best practices
Home Health care principles and guidelines
EMR system proficiency (HCHB - HomeCare HomeBase)
Strong communication and interpersonal skills
Ability to work independently and as part of a team
