Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Provide high-quality physical therapy services to patients in their homes across four counties
Conduct Oasis Assessments and Start of Care visits to develop personalized care plans
Help elderly patients recover from falls and fractures, restoring independence and quality of life
Collaborate with a multidisciplinary team to ensure coordinated patient care
Document patient progress and visit details using a tablet system

Qualifications

Active Physical Therapist (PT) license in Indiana
Experience with Oasis Assessments and Start of Care
Comfortable working in rural and mid-sized town settings
Valid driver's license and reliable transportation

Knowledge & Skills

Expertise in Oasis documentation and assessment
Strong clinical reasoning for home health settings
Excellent communication and patient education skills
Ability to manage a caseload of 4-6 patient visits per day
