Roles & Responsibilities

Provide compassionate, patient-centered nursing care consistent with LPN scope of practice
Administer medications and treatments accurately and document per facility policy
Perform timely patient assessments and report changes in condition to the RN or provider
Maintain clear, thorough documentation in the patient chart and electronic health record
Collaborate with the interdisciplinary care team to develop and follow patient care plans
Support admissions, discharges, and safe patient transfers with attention to detail
Assist in patient and family education to promote recovery, safety, and wellness

Qualifications

Current, active LPN license in Pennsylvania (required)
Current BLS/CPR certification or willingness to obtain upon hire
Comfortable working in long-term care or skilled nursing environments
Clear background check and drug screen as required by the facility
Strong communication skills and a team-oriented mindset
Previous clinical experience preferred; new grads considered with relevant training

Education

Graduate of an accredited Practical Nursing (LPN/LVN) program

Knowledge & Skills

Fundamental nursing assessments and vital sign monitoring
Medication administration and basic wound care
Familiarity with charting and documentation best practices
Infection control and patient safety principles
Ability to collaborate with nurses, aides, therapists, and providers
Time management and prioritization in a busy clinical setting
