Roles & Responsibilities

Provide compassionate, hands-on care to residents in a skilled nursing setting
Administer medications and treatments accurately and document per policy
Monitor patient condition and report changes promptly to RN/supervisor
Assist residents with activities of daily living while promoting dignity and independence
Maintain accurate clinical documentation and follow facility protocols
Collaborate with the interdisciplinary care team to support individualized care plans

Qualifications

Active LPN credential (Licensed Practical Nurse) — required
Ability to work 8-hour shifts as scheduled at the Pittsburgh facility
Strong communication and patient-centered care mindset
Comfortable working in a skilled nursing/long-term care environment

Education

Completion of an accredited LPN/LVN program (diploma or certificate)

Knowledge & Skills

Foundational nursing care and medication administration
Patient observation, vital signs, and documentation best practices
Infection control and safe care procedures
Team collaboration and clear communication with patients and families
Professionalism, reliability, and attention to detail
