Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents with professionalism and warmth
- Administer medications accurately and document administrations in the chart
- Perform routine assessments and promptly communicate changes in condition to the care team
- Collaborate with nursing staff and interdisciplinary team members to implement care plans
- Maintain clear, accurate documentation to support continuity of care and compliance
- Support residents with activities of daily living while promoting independence and dignity
Qualifications
- Valid LPN license in Pennsylvania (required)
- Graduate of an accredited LPN program
- Current BLS/CPR certification preferred or willingness to obtain
- Strong communication skills and the ability to work well within a team
- Prior experience in a long-term care or skilled nursing setting is a plus
Education
- Graduate of an accredited Practical Nursing (LPN) program or equivalent diploma
Knowledge & Skills
- Skilled in medication administration and basic nursing procedures
- Proficient in resident assessment and recognizing changes in condition
- Comfortable with documentation and charting to maintain clear clinical records
- Ability to prioritize tasks, multitask in a busy environment, and maintain calm under pressure
- Personable approach to resident care with strong interpersonal skills
