Roles & Responsibilities

• Provide compassionate, resident-centered nursing care on the long term care units
• Administer medications and treatments safely and accurately
• Perform thorough resident assessments and document findings in PointClickCare
• Monitor and respond to changes in resident condition and escalate when needed
• Serve as the unit charge LPN when assigned, coordinating workflow and staff
• Collaborate with interdisciplinary team members to support care plans
• Support infection control, safety practices, and facility policies at all times
• Float between two floors as needed to ensure consistent, quality coverage

Qualifications

• Current Licensed Practical Nurse (LPN) license required
• Minimum 2 years of recent long term care (LTC) nursing experience
• BLS certification required
• Hands-on experience with PointClickCare (PCC) or similar EMR
• Comfortable with night shift responsibilities and floating between units
• Able to take on charge nurse duties when assigned

Education

• Licensed Practical Nurse (LPN) diploma/certificate and active state licensure

Knowledge & Skills

• Strong knowledge of long term care nursing practices and documentation
• Proficiency with PointClickCare EMR for charting and medication administration
• Skilled in medication administration, wound care basics, and resident assessments
• Excellent communication and teamwork skills for collaborating with nursing staff and families
• Ability to prioritize, triage, and respond calmly in urgent situations
• Familiarity with infection control and safety standards in LTC settings
