Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents as an LPN
Administer medications and document accurately according to policy
Monitor patient conditions and report changes to RN or provider
Assist with ADLs and promote resident dignity and independence
Collaborate with the care team to create and follow care plans
Maintain clear, timely charting and communicate with families

Qualifications

Current LPN license in good standing (PA license preferred)
Current CPR/BLS certification or willingness to obtain
Ability to pass background check and required health screenings
Strong communication and teamwork skills
Previous long-term care or acute care experience preferred but not required

Education

Diploma or certificate from an accredited LPN/Vocational Nursing program
Current state LPN license

Knowledge & Skills

Familiarity with medication administration and documentation
Basic wound care and vital signs monitoring
Comfortable using electronic charting systems
Good observation skills and clinical judgment
Patient-centered approach and strong interpersonal skills
