Roles & Responsibilities

Provide compassionate direct patient care, monitoring patients and reporting changes promptly
Administer medications and document treatments accurately according to facility policy
Assist with patient assessments, basic procedures, and care plan implementation
Collaborate with RNs, physicians, and interdisciplinary team members to support quality outcomes
Maintain accurate and timely documentation in the electronic health record
Support patient and family education with clear, empathetic communication

Qualifications

Active LPN license in Pennsylvania or eligibility to obtain licensure
Current CPR/BLS certification (or willingness to obtain)
Comfortable working in a fast-paced, patient-focused environment
Strong communication skills and the ability to work collaboratively with clinical teams
Reliable, professional, and dedicated to delivering compassionate care

Education

Practical Nursing diploma or equivalent training required

Knowledge & Skills

Medication administration and basic nursing procedures
Patient assessment and vital sign monitoring
Electronic health record (EHR) documentation
Infection control practices and patient safety principles
Time management and the ability to prioritize patient needs
