Roles & Responsibilities

• Provide compassionate bedside care and assist residents with daily living activities
• Administer medications and treatments accurately and safely
• Monitor and record vital signs, changes in condition, and communicate findings to the care team
• Document care clearly and promptly to ensure continuity and quality
• Collaborate with nurses, therapists, and physicians to support individualized care plans
• Maintain infection control standards and a safe environment for residents
• Support admissions, transfers, and discharges as needed with professionalism
• Build rapport with residents and families to deliver person-centered care

Qualifications

• Current, valid LPN license in New York (required)
• Basic Life Support (BLS/CPR) certification (or willingness to obtain)
• Current immunizations and ability to pass required health screenings
• Clean background check and any facility-required screenings
• Compassionate attitude, strong communication skills, and teamwork mindset

Education

• Completion of an accredited LPN/LVN program or vocational nursing certificate
• High school diploma or GED equivalent

Knowledge & Skills

• Medication administration and basic clinical skills
• Patient assessment and monitoring (vitals, condition changes)
• Accurate documentation and charting practices
• Safe transfer and mobility assistance techniques
• Infection prevention and control principles
• Effective communication with patients, families, and interdisciplinary teams
• Time management and ability to prioritize tasks during an 8-hour shift
