Roles & Responsibilities

Provide hands-on nursing care for clinic patients within the scope of LPN licensure, ensuring high-quality, compassionate service.
Take and record vital signs, perform focused patient assessments, and triage patient calls to support timely care.
Manage medication refills and complete prior authorizations to keep patient treatments on track.
Serve as a primary liaison between patients, providers, and clinic staff to coordinate care and follow-up.
Provide direction and oversight to CNAs and Schedulers, supporting an efficient care team.
Perform accurate clinical documentation to maintain clear, compliant patient records.
Collaborate with providers, coordinators, clinical managers/supervisors, and RNs to meet patient needs and clinic goals.

Qualifications

Graduate of an accredited nursing program (LPN)
Current State LPN license (active and unencumbered)
Basic Life Support (BLS) certification
Experience taking vital signs, triaging patient calls, and managing medication refills
Familiarity with prior authorization processes and clinical documentation
Comfort supervising CNAs and coordinating with interdisciplinary teams

Education

Graduate of a Nursing Program (LPN)

Clinical documentation and charting
Basic Life Support (BLS)
