Roles & Responsibilities

• Deliver technical nursing care to residents using the nursing process under RN supervision, making a meaningful daily difference in resident comfort and safety
• Administer medications and treatments accurately and document all care in the medical record
• Observe and assess resident conditions, escalate changes to the RN, and contribute to individualized care plans
• Assume charge nurse responsibilities as needed, coordinating the nursing team and ensuring continuity of care in the SNF
• Communicate clearly and compassionately with residents, families, and interdisciplinary team members to support holistic care
• Use clinical judgment to prioritize tasks, plan work, and maintain flexibility in a dynamic care environment
• Maintain accurate electronic and written documentation and demonstrate computer proficiency for charting and care coordination

Qualifications

• Current California LVN license (required)
• Current CPR certification
• Graduate of an approved LVN program; Associate degree or equivalent preferred
• At least 1 year of experience as an LVN, SNF experience preferred
• Comfortable functioning under RN supervision and able to assume charge nurse duties when needed
• Proficient with computers and strong written and verbal communication skills

Education

• Graduate from an approved LVN program
• Associate degree or equivalent preferred

• Nursing process and resident-centered care
• Strong clinical judgment and the ability to prioritize care
• Experience in skilled nursing facility (SNF) settings
• Ability to function effectively under RN supervision
• Charge nurse capability and team coordination
• Proficient written and verbal communication
• Computer proficiency for electronic charting and documentation
• Flexibility, time management, and the ability to plan work under changing conditions
