Roles & Responsibilities

• Provide direct, compassionate patient care with attention to dignity and safety
• Administer medications and treatments accurately, following facility protocols
• Monitor and record vital signs and clinical observations, escalating concerns promptly
• Document patient care thoroughly and clearly in the chart/EHR
• Collaborate with RNs, therapists, and the interdisciplinary team to deliver coordinated care
• Assist with admissions, discharges, and patient transfers to ensure smooth transitions
• Support patients and families with education, empathy, and clear communication

Qualifications

• Active LPN license in Pennsylvania (or eligibility to obtain PA licensure)
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Comfortable administering medications and providing hands-on nursing care
• Preferred: prior LPN experience in long-term care, rehab, or acute care settings
• Strong communication skills and ability to work collaboratively in a fast-paced environment

Education

• LPN diploma, certificate, or equivalent practical nursing program completion

Knowledge & Skills

• Medication administration and documentation best practices
• Ability to perform accurate vital sign assessments and patient monitoring
• Familiarity with electronic health records (charting proficiency is a plus)
• Basic wound care, ADL assistance, and patient mobility support
• Excellent interpersonal skills and patient/family education techniques
