Roles & Responsibilities

Deliver compassionate, patient-centered nursing care to residents in a skilled nursing setting
Administer medications and treatments accurately and document thoroughly
Monitor resident condition and communicate changes to the interdisciplinary team
Assist with activities of daily living while promoting resident dignity and independence
Participate in care planning and contribute clinical observations to support positive outcomes
Provide family-centered communication and education with empathy and clarity

Qualifications

Current, active LPN license in New York (required)
Valid CPR/BLS certification (preferred)
Comfortable working in a long-term care/skilled nursing environment
Strong communication skills and the ability to work well on a multidisciplinary team
Professional, dependable, and patient-focused approach to care

Education

Vocational Nursing Diploma or Practical Nursing Certificate (required)

Knowledge & Skills

Skilled in medication administration and documentation
Basic assessment skills and clinical observation
Familiarity with long-term care regulations and best practices
Good infection control and safety practices
Strong time management and prioritization in a busy clinical setting
