Contract Type: Per-Diem

Shift Type: 8.5-hour shift (schedule to be confirmed; days may vary) - LPN coverage

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents as an LPN
Administer medications and document responses accurately and promptly
Monitor patient status and report changes to the care team to ensure safe outcomes
Assist with care planning and collaborate with nurses and interdisciplinary staff
Deliver excellent patient-centered communication with residents and families
Maintain accurate clinical records and follow facility policies and procedures

Qualifications

Active LPN license in Pennsylvania (required)
Graduate of an accredited practical nursing program or equivalent
Current Basic Life Support (BLS/CPR) certification preferred or obtained upon hire
Demonstrated ability to work in long-term/post-acute care or similar setting is a plus
Strong clinical judgment, reliable attendance, and a caring bedside manner

Education

High school diploma or equivalent required
Completion of an accredited Practical Nursing program (LPN certificate/diploma)

Knowledge & Skills

Medication administration and documentation best practices
Wound care basics and vital signs assessment
Infection control and standard precautions
Effective communication with patients, families, and interdisciplinary teams
Time management and prioritization in a busy clinical environment
