Roles & Responsibilities

• Provide compassionate direct patient care and support daily living activities
• Administer medications and treatments according to physician orders and facility policy
• Monitor and record vital signs, patient status, and changes in condition promptly
• Collaborate with nurses, therapists, and the interdisciplinary team to ensure excellent care
• Accurately document patient care in the chart and maintain up-to-date records
• Educate patients and families on care plans and self-care techniques in a clear, respectful way
• Assist with wound care, catheter care, and other assigned nursing tasks as delegated
• Maintain a safe, clean environment and follow infection control and safety protocols

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania (required)
• Current Basic Life Support (BLS/CPR) certification or willingness to obtain
• Comfortable administering medications and performing routine nursing procedures
• Strong communication and teamwork skills with a patient-centered mindset
• Ability to pass background check, drug screen, and meet facility onboarding requirements

Education

• Completion of an accredited LPN/Vocational Nursing program
• High school diploma or GED

Knowledge & Skills

• Clinical assessment and basic nursing interventions
• Medication administration and documentation best practices
• Familiarity with electronic health records or charting systems
• Infection control, patient safety, and compliance with facility policies
• Time management and prioritization in a fast-paced care environment
• Effective communication with patients, families, and interdisciplinary teams
