Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents in a long-term care setting
• Administer medications and treatments accurately and document according to facility policy
• Monitor patient condition, report changes to the care team, and escalate concerns promptly
• Collaborate with CNAs, RNs and interdisciplinary team members to develop and follow individualized care plans
• Educate residents and families about care plans, medications and daily routines in a clear, respectful way
• Maintain accurate and timely clinical documentation to support continuity of care and compliance
• Support a safe, clean environment and follow infection control and safety protocols

Qualifications

• Current LPN license in Pennsylvania (or compact state authorization where applicable)
• Active Basic Life Support (BLS/CPR) certification or willingness to obtain
• Previous experience in long-term care, skilled nursing or a related clinical setting preferred
• Strong communication and teamwork skills; dependable and compassionate bedside manner
• Ability to stand, walk and perform hands-on patient care for scheduled shift lengths

Education

• Graduate of an accredited Practical Nursing program (LPN diploma or certificate)

Knowledge & Skills

• Medication administration and documentation best practices
• Fundamental nursing assessments and vital sign interpretation
• Familiarity with long-term care workflows and common therapies
• Electronic health record (EHR) documentation comfort — basic computer skills
• Time management and prioritization in a fast-paced care environment
