Roles & Responsibilities

• Provide compassionate, hands-on patient care including medication administration and personal care
• Perform timely and accurate documentation of patient status and interventions
• Collaborate with nurses and interdisciplinary team members to support patient-focused care plans
• Monitor patient condition and report changes promptly to ensure safety
• Assist with procedures and nursing tasks while following facility protocols and infection control standards
• Support patients and families with education, reassurance, and clear communication

Qualifications

• Current Licensed Practical Nurse (LPN) license in New Hampshire or eligible to obtain state licensure
• Current Basic Life Support (BLS) or CPR certification preferred or willing to obtain
• Comfortable working day shifts and adapting to per-diem scheduling
• Strong communication skills and a patient-centered attitude
• Ability to pass facility-required screenings and background checks

Education

• Completion of an accredited LPN program or equivalent practical nursing certificate/diploma

Knowledge & Skills

• Solid fundamentals in medication administration and basic nursing procedures
• Good clinical assessment and observation abilities
• Accurate charting and electronic documentation skills
• Ability to prioritize tasks in a fast-paced clinical setting
• Friendly bedside manner and effective teamwork skills
