Location: Lebanon, New Hampshire

Contract Type: Per-Diem

Shift Type: Per diem (days as assigned), 7:00 AM - 7:30 PM, Day shift (AP)

Hours per Week: 36

Candidate Pay Rate: $36 per hour

Roles & Responsibilities

Provide compassionate direct patient care consistent with LPN scope of practice
Administer medications and treatments safely and accurately
Perform focused patient assessments and communicate changes to the care team
Document care in the medical record clearly and timely
Support patient comfort, hygiene, and activities of daily living
Collaborate with RNs, physicians, and interdisciplinary staff to implement care plans
Assist with admissions, discharges, and patient education to promote positive outcomes

Qualifications

Valid Licensed Practical Nurse (LPN) license in New Hampshire (required)
Current basic life support (BLS) certification, or ability to obtain as required by facility
Comfortable working in a fast-paced clinical environment with strong communication skills
Professional, dependable, and patient-centered with good teamwork skills

Education

Graduate of an accredited practical nursing (LPN) program

Knowledge & Skills

Strong fundamentals in medication administration and basic nursing procedures
Patient assessment and vital signs monitoring
Effective clinical documentation and charting skills
Ability to follow care plans and escalate changes appropriately
Interpersonal skills for working with patients, families, and the care team
