Roles & Responsibilities

Provide direct nursing care to clinic patients within the scope of LPN licensure, following established standards and best practices.
Perform patient assessments, take vital signs, and triage patient calls to prioritize care and ensure patient safety.
Manage medication tasks including refills and medication management communications with providers.
Complete prior authorizations and other administrative clinical tasks to keep patient care moving smoothly.
Serve as the primary liaison between patients, providers, and clinic staff to coordinate care and resolve issues.
Delegate appropriate tasks to CNAs and schedulers while working under the direction of RNs, providers, and clinical managers.
Document patient encounters and care accurately and timely in the medical record.
Educate patients on condition management, medications, and follow-up care to improve outcomes and satisfaction.

Qualifications

Current State LPN license in good standing
Basic Life Support (BLS) certification required
Graduate of a Nursing Program
Comfortable triaging patient calls and performing accurate vital sign assessments
Experience with medication refills and prior authorization workflows
Strong communication skills and ability to work collaboratively with interdisciplinary teams

Education

Graduate of a Nursing Program (LPN)

Knowledge & Skills

Patient assessment and clinical nursing fundamentals
Triage protocols and telephone triage communication
Medication management and refill procedures
Prior authorization processes and payer communications
Clear, professional documentation practices
Patient education and coaching
Ability to delegate tasks to CNAs and coordinate with schedulers
Interdisciplinary teamwork and collaboration
