Roles & Responsibilities

Provide compassionate, hands-on patient care and daily living support
Administer medications and treatments accurately and document care thoroughly
Collaborate with nurses and interdisciplinary team members to create patient-focused plans
Monitor patient condition and report changes promptly to clinical leadership
Maintain accurate medical records and ensure regulatory compliance
Support a positive environment by mentoring CNAs and communicating with families

Qualifications

Active LPN license or practical nurse certification (LPN)
Eligibility for Connecticut state licensure or current CT LPN license
Current Basic Life Support (BLS/CPR) certification or willingness to obtain
Prior experience in long term care or skilled nursing preferred
Strong communication skills and a patient-centered attitude

Education

Completion of an accredited LPN/Licensed Practical Nurse program
High school diploma or equivalent

Knowledge & Skills

Clinical assessment and basic nursing procedures
Medication administration and documentation best practices
Familiarity with electronic health records and point-of-care charting
Infection control, safe patient handling, and fall prevention
Team collaboration and clear communication with patients and families
