Roles & Responsibilities

• Coordinate patient care across the interdisciplinary team to support safe, timely discharges
• Develop and implement individualized plans of care in collaboration with physicians, nursing, and families
• Perform discharge planning and arrange DME, SNF, and ambulance transport independently
• Conduct needs assessments and manage prior authorizations using evidence-based guidelines
• Utilize InterQual and other medical necessity tools to determine appropriate levels of care
• Document thoroughly and accurately in Epic to ensure continuity and compliance
• Organize and participate in family meetings and contribute to treatment plan development
• Support utilization review, data abstraction, and regulatory requirements to maintain quality standards

Qualifications

• Minimum 2 years acute hospital case management experience performing care coordination and discharge planning
• Experience managing patient loads consistent with a 1:20 ratio
• Charting experience in Epic is required
• Travel experience required (travel clinicians welcomed; locals accepted)
• BLS preferred
• Ability to float between like units as needed and adapt in a fast-paced environment
• First-time travel clinicians may be screened with Clinical approval

Knowledge & Skills

• Strong knowledge of care coordination, discharge planning, and admission criteria
• Familiarity with InterQual criteria and utilization review processes
• Experience with prior authorizations, benefits eligibility, and DME/SNF/transport arrangements
• Data abstraction, ICD-10 and CPT coding knowledge for accurate documentation
• Understanding of CMS, DRG, HIPAA, NCQA, OSHA and The Joint Commission requirements
• Comfort working across acute, ambulatory, behavioral health, home health, hospice and long-term acute care settings
• Excellent communication and family meeting facilitation skills
• Proficient with computerized documentation and Epic workflows
