Roles & Responsibilities
- Provide compassionate direct patient care consistent with LPN scope of practice
- Administer medications and treatments safely and accurately
- Perform focused patient assessments and communicate changes to the care team
- Document care in the medical record clearly and timely
- Support patient comfort, hygiene, and activities of daily living
- Collaborate with RNs, physicians, and interdisciplinary staff to implement care plans
- Assist with admissions, discharges, and patient education to promote positive outcomes
Qualifications
- Valid Licensed Practical Nurse (LPN) license in New Hampshire (required)
- Current basic life support (BLS) certification, or ability to obtain as required by facility
- Comfortable working in a fast-paced clinical environment with strong communication skills
- Professional, dependable, and patient-centered with good teamwork skills
Education
- Graduate of an accredited practical nursing (LPN) program
Knowledge & Skills
- Strong fundamentals in medication administration and basic nursing procedures
- Patient assessment and vital signs monitoring
- Effective clinical documentation and charting skills
- Ability to follow care plans and escalate changes appropriately
- Interpersonal skills for working with patients, families, and the care team
