Location: Lebanon, NH

Contract Type: Per-Diem

Shift Type: Per diem (varies by assignment), 3:00 PM - 11:00 PM, Evening shift

Hours per Week: 40

Candidate Pay Rate: $22 per hour

Roles & Responsibilities

Provide compassionate, hands-on care to residents, supporting activities of daily living
Assist with bathing, grooming, dressing, feeding, and mobility with dignity and respect
Monitor residents and report any changes in condition to the nursing team
Document care accurately and promptly to support safe, coordinated treatment
Maintain a clean, safe environment and follow infection prevention practices
Collaborate with team members to deliver person-centered care and positive outcomes

Qualifications

Active Licensed Nursing Assistant (LNA) credential (VT or NH licensure preferred given regional coverage)
Completion of an approved LNA program
Current basic life support (BLS) preferred where required
Ability to pass required background checks and health screenings
Reliable, compassionate, and able to communicate effectively with residents and team members

Education

High school diploma or equivalent and completion of an approved LNA program

Knowledge & Skills

Strong knowledge of activities of daily living (ADLs) and resident assistance techniques
Basic understanding of infection control and safety procedures
Comfortable with routine documentation and care reporting
Good communication skills for working with residents, families, and interdisciplinary staff
Team-oriented mindset and ability to follow care plans and supervisor directions
