Roles & Responsibilities
- Provide compassionate bedside care and maintain high standards of patient safety
- Administer medications and treatments accurately and document all care activities
- Monitor vital signs and clinical status, escalating changes to the care team as needed
- Perform wound care, ADLs assistance, and other nursing tasks within LPN scope
- Collaborate with RNs, physicians, and interdisciplinary team members to deliver cohesive patient care
- Educate patients and families about care plans, discharge instructions, and health maintenance
- Maintain clear, timely clinical documentation to support continuity of care
Qualifications
- Current Licensed Practical Nurse (LPN) license — active and in good standing
- Minimum clinical experience in long-term care or skilled nursing preferred
- Current BLS/CPR certification (or willing to obtain prior to start)
- Comfortable with medication administration, documentation, and basic wound care
- Ability to pass facility background checks and meet health/occupational requirements
- Professional, reliable, and able to thrive in a team-oriented environment
Education
- Vocational nursing diploma or equivalent required; LPN licensure required
Knowledge & Skills
- Strong clinical assessment skills and attention to detail
- Excellent communication and interpersonal skills with patients and families
- Proficiency in electronic documentation and charting best practices
- Time management and prioritization for safe delivery of care during long shifts
- Ability to work effectively within interdisciplinary care teams
