Roles & Responsibilities

• Provide compassionate bedside care and maintain high standards of patient safety
• Administer medications and treatments accurately and document all care activities
• Monitor vital signs and clinical status, escalating changes to the care team as needed
• Perform wound care, ADLs assistance, and other nursing tasks within LPN scope
• Collaborate with RNs, physicians, and interdisciplinary team members to deliver cohesive patient care
• Educate patients and families about care plans, discharge instructions, and health maintenance
• Maintain clear, timely clinical documentation to support continuity of care

Qualifications

• Current Licensed Practical Nurse (LPN) license — active and in good standing
• Minimum clinical experience in long-term care or skilled nursing preferred
• Current BLS/CPR certification (or willing to obtain prior to start)
• Comfortable with medication administration, documentation, and basic wound care
• Ability to pass facility background checks and meet health/occupational requirements
• Professional, reliable, and able to thrive in a team-oriented environment

Education

• Vocational nursing diploma or equivalent required; LPN licensure required

Knowledge & Skills

• Strong clinical assessment skills and attention to detail
• Excellent communication and interpersonal skills with patients and families
• Proficiency in electronic documentation and charting best practices
• Time management and prioritization for safe delivery of care during long shifts
• Ability to work effectively within interdisciplinary care teams
