Roles & Responsibilities
- Evaluate residents to establish individualized, functional treatment plans
- Develop long- and short-term therapeutic goals and document them clearly
- Deliver therapeutic interventions and modalities to maximize independence
- Document all evaluations and treatments in the Electronic Medical Record
- Recommend, fit, and instruct residents on appropriate assistive devices
- Educate residents and caregivers and document teaching outcomes
- Collaborate with the interdisciplinary care team and attend team meetings
- Supervise Physical Therapist Assistants and mentor new staff during orientation
- Ensure compliance with Medicare, Medicaid, and other reimbursement requirements
- Provide in-service education and help develop new rehabilitation programs
- Maintain department cleanliness and follow infection control procedures
Qualifications
- Active Physical Therapy (PT) license in the state of Massachusetts (required)
- Bachelor's degree required; Master's or Doctorate in Physical Therapy preferred
- Basic Life Support (BLS) certification preferred
- Prior experience in long-term care or skilled nursing facility preferred
- Demonstrated ability to supervise assistant-level staff appropriately
- Strong verbal and written communication, documentation, and time management skills
- Ability to perform physical tasks including lifting up to 50 pounds and assisting residents
Education
- Bachelor's degree required
- Master's or Doctorate in Physical Therapy preferred

- Patient evaluation and treatment plan development
- Therapeutic interventions and modalities
- EMR documentation best practices
- Assistive device recommendation and training
- Interdisciplinary team collaboration and resident-centered care
- Knowledge of Medicare, Medicaid, and reimbursement regulations
- Proficiency in Microsoft Office and basic computer skills
- Teaching and in-service presentation skills
