Location: Andover, MA

Contract Type: Per-Diem

Shift Type: Per diem (as scheduled), 3:00 PM - 11:00 PM, Evening shift

Hours per Week: 40

Candidate Pay Rate: $45 per hour

Roles & Responsibilities

Provide compassionate, safe direct patient care to residents in a skilled nursing/rehab setting
Administer medications and treatments per orders and facility policies
Assess and document patient status, changes, and care plans in the electronic chart
Collaborate with nurses, therapists, and support staff to deliver coordinated care
Assist with admissions, discharges, and timely patient transfers
Maintain infection control practices and ensure a safe care environment
Communicate with residents and families with empathy and professionalism

Qualifications

Current Licensed Practical Nurse (LPN) license in good standing
Previous experience in long-term care, skilled nursing, or rehabilitation is a plus
Basic Life Support (BLS/CPR) certification preferred or willing to obtain
Ability to work evening per-diem shifts (3:00 PM - 11:00 PM) and be flexible with scheduling
Strong communication skills and a team-oriented mindset

Education

Graduate of an accredited Practical Nursing program
High school diploma or equivalent

Knowledge & Skills

Skilled in medication administration and resident assessments
Comfortable with charting and electronic health records
Experience with wound care and basic restorative techniques
Excellent time management and prioritization in a busy environment
Effective interpersonal skills for working with residents, families, and the care team
