Roles & Responsibilities

• Provide skilled physical therapy evaluation and evidence-based treatment to SNF residents
• Develop individualized care plans focused on functional gains and safe discharge
• Collaborate with interdisciplinary team members to optimize patient outcomes
• Document progress, outcomes, and skilled rationale clearly and timely
• Coach residents and families on fall prevention, home exercise programs, and mobility safety
• Participate in required facility meetings and quality improvement efforts

Qualifications

• Licensed Physical Therapist (PT) or Doctor of Physical Therapy (DPT); ability to obtain Arizona licensure if not already licensed
• New graduates welcome; all experience levels considered
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Comfort working in a skilled nursing facility and collaborating with nursing staff
• Strong communication skills and patient-centered bedside manner

Education

• Doctor of Physical Therapy (DPT) preferred; or completion of an accredited PT program

Knowledge & Skills

• Skilled in mobility training, transfers, gait training, and therapeutic exercise
• Knowledge of documentation standards for SNF and Medicare/Medicaid considerations
• Ability to perform safe patient handling and fall-risk assessment
• Familiarity with interdisciplinary care planning and rehab-focused discharge planning
• Good clinical reasoning and outcome-driven treatment planning
